Ukraine's role as Euro 2012 co-host is in jeopardy amid a row over government interference in the country's football authorities.
"If the FFU [Football Federation of Ukraine] is suspended from membership of UEFA, it would not be tenable to play the final round of the UEFA European football championship in Ukraine in 2012," UEFA general secretary Gianni Infantino said in a letter to the FFU.
Ukraine has been given until February 4 to give a satisfactory response.
The row erupted last year after international footballing officials got hold of documents indicating pressure on the FFU from Ukrainian authorities.
A suspension would mean that Ukrainian clubs and national team would have to leave European competitions.
"A suspension means that the clubs cannot take part in our competitions, and we have still three clubs, one in the Champions League and two in the Europa League," Infantino told reporters on Friday.
"The national teams would also be suspended from official competitions, and as a concrete step, as a preventative we have decided to place the Ukrainian under-21 team in a group of six (for qualifying)," he said.
Euro 2012 will be held in four Polish and four Ukrainian cities. Ukraine's readiness to hold the 2012 tournament has been a constant source of concern for UEFA.
