2012 Summer Olympics in London
During the Olympic torch relay the flame was being carried by members of the UK rafting team. Splashes of water and a strong head wind put the flame out and the team had to come ashore and wait for the arrival of special capsules with the Greek fire inside.
2014 Winter Olympics in Sochi
The Olympic torch was being carried by USSR scuba diving champion Shavarsh Karapetyan, famous for having saved dozens of people during accidents. When the flame suddenly went out, Shavarsh, unfazed by the mishap, asked a local security guard to rekindle the flames with his lighter.

During the ceremony to light the Olympic cauldron the skies over Montreal opened up with the driving rain quickly putting out the Olympic flame.
1988 Summer Games in Seoul
A total of 2,400 white pigeons set free to soar over the stadium chose what they apparently thought was the best seat in the house – the rim of the Olympic flame cauldron.
Moments later most of them were roasted when the fire ignited.
