The US State and Defense Departments need to improve their vetting of Iraqi security forces personnel, the Government Accountability Office (GAO) said in a report.

WASHINGTON (Sputnik) — The GAO also found that, as of September 2016, the State Department had not updated its human rights vetting policy to reflect the most recent changes to Defense Department law requiring vetting for training, provision of equipment and other assistance.

"DOD [Department of Defense… record keeping of vetted units and Iraq security force personnel was not accurate or reliable," the report said on Tuesday. "We recommended that State update its agency-wide vetting policy, and that DOD improve its vetting records."

"The agencies generally agreed with our recommendations and have taken steps to implement them," the report acknowledged.

The State Department had informed the GAO that it had now updated its human rights vetting policy to reflect changes in the Defense Department law and posted the law’s text on its internal department network, the report added.
